How to get a refund from fisher price for rock and play

The fisher-price rock and play has been recalled. Recall notice here. Summary: stop using under any circumstances.  It's annoying to find the page for a refund/exchange from their website. So, here it is: https://service.mattel.com/us/recall/BJD57_ivr.asp

You have to send them back the two "hubs." It's best if you have a receipt, but at the very worst you will get a voucher from fisher-price for some baby products.
